Begin the steps to obtain a service dog.

Here is how it works:

  • Your primary medical diagnosis must impact your mobility. Our dogs are not trained to assist with seizures, diabetes, psychological/emotional, PTSD, POTS, or autism.

  • Live within the Chicagoland area.

  • Be between the ages of 13-70.

  • Be willing to attend our Monday night classes every week for at least 1 year.

  • A mandatory home visit is required for all applicants prior to acceptance.

  • A medical form completed by your physician is required (after in-person interview).

  • Have patience as this can be a long process.

  • The first step in our application process is completing the below inquiry. We initially ask for basic information about your needs and eligibility, which allows us to better understand how a service dog could potentially support you. Completing the inquiry form does not guarantee approval, enrollment in the program, or a service dog in the future. It simply helps us assess your situation and determine if a service dog is the right solution for your needs. After reviewing your inquiry, we may reach out for further clarification or to discuss next steps, which may include completing an application, providing additional documentation, or undergoing a more detailed assessment before any decisions can be made.
